In Ontario, you have 3 months from the service date to submit your claims. Claims will not be accepted for payment unless there are extenuating circumstances as defined by ministry policy.
To submit stale claims (claims over 3 months old) you must first send a written request with supporting documentation to the claims services Branch Manager at ClaimsManagement@ontario.ca.
Once you've received written approval from the Manager, you can create your claims and in the Advanced Tab change the Stale Dated Claim indicator to "YES".
Once you have submitted your Stale Dated Claims, you will need to contact your Branch Manager to inform them of the submission.
